BAPTISM REGISTRATION

Baptism Registration (For children 7 years of age or younger)

Godparent Baptismal Form

Policy: Parents must be a registered parishioner of St. Frances Cabrini or one of our Quo Vadis Sister Parishes before a date can be reserved. Once you submit your request for a baptism, please email child’s birth certificate and the signed/filled godparent forms to mramos@qvdomine.org in order to start the baptism process.

PRE-CANA REGISTRATION

Pre-Cana Registration

We look forward to welcoming you as you prepare for the beautiful Sacrament of Marriage! Please note that space for each Pre-Cana retreat is limited, and registrations are accepted on a first-come, first-served basis. Your registration is considered complete and your place is confirmed only after both the completed registration form and payment in full have been received.To help us prepare well for each retreat, all registration forms and full payments must be received no later than the Monday prior to the retreat. We encourage couples to register early to ensure availability.
MARRIAGE REQUEST FORM

Marriage Request Form

Policy: Bride or groom must be a registered and active parishioner for 6 months, or the child of one, before a date can be reserved. If eligible to reserve a date: please note, this is only a request form. Confirmed date/time will occur once approved by the priest. Do not make any plans, until you receive confirmation.
